You will have to submit this form 2 times each month, each time stating your work hours for either:
- The period from 1st → 15th; or
- The period from 16th → end the month
Remember to submit preferably right after you see this email, or two days prior your pay-check date. This helps your Account Team to ensure your payments be settled on time.
